关节镜下缝线固定前交叉韧带止点撕脱骨折

Arthroscopic treatment of tibial eminence fracture with suture fixation technique

目的探讨关节镜下缝线固定治疗前交叉韧带止点撕脱骨折的临床效果。方法对15例新鲜的明显移位前交叉韧带止点撕脱骨折在关节镜下进行复位，使用不吸收Ethicon缝线进行固定。术后进行积极康复训练。结果随访6～24个月，术后6周所有骨折均得到愈合，无骨折移位出现。术后3个月，无膝关节松弛或者不稳定发生，所有患者都恢复了与健侧相同的活动度。术后6个月，Lyscholm膝关节功能评分（94．1±2．5）分。结论关节镜下缝线固定治疗前交叉韧带止点撕脱骨折，复位和固定效果好，创伤小，早期进行膝关节康复训练，能够最大程度恢复膝关节功能。

Objective To evaluate the clinical results of arthroscopic treatment of tibial eminence fracture with suture fixation technique. Methods 15 cases of tibial eminence fracture were treated arthroscopically with non-absorbable Ethicon suture fixation technique, Active rehabilitation was begun after operation. Results The patients were followed up for 6 - 24 months, All fractures healed without displacement six weeks after operation. Three months postoperatively, no knee laxity or instability was detected in all patients;full range of motion of the affected knee was restored in all patients compared with the normal side. Six months postoperatively,the Lysholm knee score was（ 94. 1 ± 2. 5 ）. Conclusions Arthroscopic treatment of tibial eminence fracture with suture fixation technique is reliable for fracture reduction, fixation and early mobilization. Arthroscopy is indicated to diagnose and treat associated knee arthropathy, reduce the morbidity associated with arthrotomy, and restore knee function to the greatest extent when combined with suture fixation techniques.
