摘要
目的:研究皮质下缺血性血管性痴呆(SIVD)患者听觉信息自动加工过程变化特征。方法:应用意大利EB-NEURO事件相关电位工作站,对17例SIVD患者和15例同龄对照者进行失匹配负波(MMN)成分的潜伏期、波幅检测,并进行偶极子源分析及皮层电流密度测定。结果:与对照组相比,SIVD组MMN波形欠规则,潜伏期延长,波幅明显降低,差异均有显著意义(P<0.01);SIVD患者MMN偶极子定位于右侧颞叶上回及右侧眶额叶部附近;SIVD组双侧颞区、右侧额叶外侧皮层电流密度明显减弱。结论:SIVD患者在大脑信息处理的早期阶段对所输入信息的感觉记忆及自动选择加工存在障碍,可能与听觉信息自动加工皮层下-颞叶-额叶环路受损有关。
Objective:To investigate the charaeteristics of auditoy automatie information proeessing in patients with subcortical ischemic vascular dementia(SIVD). Methods: Mismatch negativity(MMN) was elicited from 17 SIVD patients and 15 normal controls (NC), and the latencies and the amplitudes of MMN were analyzed. The dipole source of MMN, and current density were detected,as compared with NC group. Results: The formation of MMN was less regular, and the latencies (P〈0. 01 ) was prolonged and the amplitudes (P〈 0.01) decreased in SlVD group. Dipole source analysis showed two dipoles of MMN located in the right superior temporal gyrus and the right orbitofrontal cortex. Furthermore, the intensity of current density in SIVD group was attenuated. Conclusions: Sensory memory and auditory automatic information process at the early stage in patients with SIVD are impaired, which may be related to the interruption of circuit loop of subcotical- temporal- frontal lobe on auditory automatic information process.
