肉毒毒素治疗儿童痉挛型脑瘫拇指内收畸形的临床疗效分析

Application of botuliunm toxin A in spastic cerebral palsy with thumb abnormality

目的探讨A型肉毒毒素对痉挛型脑性瘫痪拇指内收畸形的疗效。方法50例脑性瘫痪患儿随机分为两组，观察组采用A型肉毒毒素肌肉注射和手传统中医按摩、拇指矫形器固定及上田法训练，对照组采用手传统中医按摩、拇指矫形器固定和上田法训练。结果经过3mo治疗，两组总有效率分别为96.0％和78．0％，观察组明显优于对照组（χ2=7．16，P〈0.01），且观察组拇指腕掌外展活动度明显改善，与对照组相比差异有显著性（t=5.20，P〈0．01）。结论A型肉毒毒素肌肉注射对脑性瘫痪拇指内收畸形的疗效较好。

Objective To study the effects of botulinum toxin A （BTX -A） in spastic cerebral palsy（CP） with thumb abnormality. Methods Fifty patients with CP were divided into two groups. The trial group was treated with BTX - A intramuscrlar injection and massotherapy, orthosis fixation, shangtian method training ,The control group was treated with massotherapy, orthosis fixation, shangtian method training. Results After three months, the total effective rate of the two groups were respectively 96.0% and 78.0%. There were clear difference between the two groups. The trial group was better than the control group（χ2 = 7.16 , P 〈 0.01 ）. And thumb range motion of the trial group was better than the control group. Conclusion It has positive effect that BTX - A intramuscrlar injection treat spastic CP with thumb abnormality.
