微卡辅助治疗老年初治菌阳肺结核临床对比观察

Effect Vaccae as Auxiliary Treatment for Sputum Positive Pulmonary Tuberculosis among Old People

目的:观察和评价微卡辅助治疗老年初治菌阳肺结核的疗效。方法:将120例老年初治菌阳肺结核病人随机分为治疗组和对照组各60例。对照组采用2HRZE/4HR方案治疗。治疗组采用上述方案治疗的同时,加用微卡22.5ug,臀部肌肉注射,每周1次,疗程2个月。然后观察痰菌阴转、肺部病灶吸收、空洞闭合等情况。结果:痰菌阴转情况:1个月后治疗组46例,阴转率76.7%,对照组32例,阴转率53.3%,两组比较有高度显著性差异(P<0.01);2个月后治疗组55例,阴转率91.7%,对照组43例,阴转率71.7%,两组比较有高度显著性差异(P<0.01);3个月后治疗组60例,阴转率100%。对照组58例,阴转率96.7%.两组比较无差异。病变吸收情况:2个月、4个月、疗程结束后显效病例,治疗组分别为27、48、52例,对照组分别为14、32、40例,两组比较分别有高度显著性差异(P<0.01)。空洞闭合情况:2个月、4个月、疗程结束后空洞闭合病例,治疗组分别为11、19、21例,对照组分别为4、13、19例,两组比较分别有显著性差异(P<0.05)。结论:微卡辅助治疗老年初治菌阳肺结核有显著性疗效。

Objective ： To observe the effect of Vaccae as auxiliary treatment for sputum positive pulmonary tuberculosis among old people. Method： 120 patients were divided into two groups,namely the case group and the control group. The control group were treated with 2HRZE/4HR,the case group were treated with 2HRZE/4HR combined with injection of Vaccae of 22.5ug, once a week for a period of two montho Observation was made on the sputum negative conversion, focus absorption and cavity clisure. Result： The sputum negative conversion is as follows： 46 cases in the cases group and 32 cases in the control group after 1 month,the difference was significant（ p 〈0.01 ） ;55 cases in the trentment group and 43 cases in the control group after 2 months,the difference was also significant（ P 〈0.01） ;60 cases in the case group and 58 cases in the control group after 3 months,the difference was not significant. The focus absorption was as follows ：27, 48 and 52 cases in the case group and 14,32 and 40 cases in the control group after 2 months,4 months and after treatment respectively （ P 〈 0.01 ）. cavity closure is as follows ： 11,19 and 21 cases in the case group and 4,13 and 19 cases in the control group after 2 month,4 months and after treatment respectively ,the difference was significant （ p 〈 0.05 ）. Conclusion： Using Vaccae as auxiliary treatment for sputum positive pulmonary tuberculosis among old people is of significant effect.
