46例合并肝硬化腹腔镜胆囊切除术的体会

Treating Experience of 46 Cases LC Complicated with Liver Cirrhosis

目的:探讨合并肝硬化的腹腔镜胆囊切除的适应症及手术技巧,总结合并肝硬化行腹腔镜胆囊切除术的治疗经验。方法:回顾分析1997年6月至2004年12月肝硬化行腹腔镜胆囊切除术46例的临床资料,总结其手术指征、术前准备以及术中操作技巧。结果:44例在腹腔镜下完成,2例因胆囊动脉出血中转开腹,无死亡、肝功能衰竭及胆管损伤等并发症。结论:加强围手术期的处理,及时中转开腹,对childA或childB级肝硬化病人配合熟练的腹腔镜手术经验及技巧,行腹腔镜胆囊切除术是可行的。

Objective： To study the surgical indication and skill of laparoscopic cholecystectomy（LC） in patients complicated with liver cirrhosis and to sum up the clinical experience of treatment.Method： From June 1997 to December 2004,46 cases with liver cirrhosis received LC.Their clinical data were analyzed retrospectively.The surgical character,prepatation of proesurgery and the skill during surgery were summed up.Result： 44 cases were cured by LC,2 cases were converted to open operation.There were no death and complications like function failure of liver or bile duct injury.Conclusion： It is very important to emphasized the perioperative management and convert to open operation in time.LC is safe and effective for treatment of patients complicated with liver cirrhosis.
