经腹超声检测原发性痛经子宫动脉血流动力学研究

Transabdominal ultrasound study of uterin artery hemodynamics in primary dysmenorrhea

目的:经腹超声(TBUS)检测未婚少女原发性痛经(Primary Dysmenorrhea,PD)子宫动脉血流动力学改变,探讨痛经程度与子宫血流灌注的相关性。方法:54名PD未婚少女和41名正常对照者在月经周期第一天经腹超声检查,记录子宫动脉、弓状动脉、放射动脉、螺旋动脉的搏动指数(PI)与阻力指数(RI),按《中药新药治疗痛经的临床研究指导原则》进行痛经积分和痛经程度判定。结果:中度组、重度组与对照组比较在月经期第一天子宫动脉较细分支血流阻力较高;螺旋动脉的搏动指数、阻力指数与痛经积分具有一定相关性。结论:经腹超声能够检测到未婚少女子宫动脉血流参数;PD月经周期第一天子宫动脉灌注阻力高;PD严重程度与螺旋动脉的搏动指数、阻力指数相关。

Objective： To determine the vasoconstriction of the uterine vessels in unmarried girls with primary dysmenorrhea by transabdominal ultrasound and the correlation between PD severity and Uterin Artery hemodynamics . Methods： fifty - four unmarried girls with primary dysmenorrhea and forty - one healthy controls were included in this prospective study. Girls were examined with transabdominal ultrasound on first day of the cycle. Measurements of pulsatility index and resistance index in uterine, areuate, radial and spiral arteries were performed According to （The guide principle of dine study to the treatment of dysmenorrhea with new Chinese traditional medicine）, we get scores and judge severity of symptoms in primary dysmenorrhea. Results： A significant difference in Doppler index values in the middle and severe dysmenorrhea groups was observed on first day of the cycle for the radial and spiral arterys. Score was correlated with PI and RI in spiral artery. Conclusion： The women with primary dysmenorrhea have elevated Doppler indices in uterin arteries on first day of the cycle. There is correlation between PD severity and Uterin Artery hemodynamics.
