膀胱移行细胞癌中FHIT、p21^(waf1/cip1)基因的表达

摘要 目的通过测定脆性组氨酸三联体基因(FHIT)及p21waf1/cip1基因在膀胱移行细胞癌组织、正常膀胱组织中的表达,探讨FHIT基因以及p21waf1/cip1基因与膀胱癌的关系及其临床意义。方法采用免疫组织化学SP法对43例膀胱移行细胞癌(BTCC)组织及14例正常膀胱组织中的FHIT基因及p21waf1/cip1基因的蛋白表达进行检测。结果Fhit蛋白与p21waf1/cip1蛋白在正常膀胱组织中的表达明显高于膀胱癌组织(P<0.05);Fhit蛋白表达与肿瘤的分期、分级无相关性(P>0.05),而p21waf1/cip1蛋白的表达与之有相关(P<0.05);Fhit蛋白的表达在GI期肿瘤、浅表性肿瘤中明显低于在正常膀胱组织中的表达(P<0.05),而p21waf1/cip1蛋白的表达在上述组织比较中差异无显著性;Fhit蛋白的表达在初发肿瘤中与复发肿瘤中差异无显著性,而p21waf1/cip1蛋白的表达在上述两种组织比较差异有显著性(P<0.05)。Fhit蛋白与p21waf1/cip1蛋白的表达没有相关性。结论FHIT基因可能成为早期诊断膀胱移行细胞癌的指标。p21waf1/cip1基因可能成为估计膀胱移行细胞癌的恶性程度及肿瘤侵袭性、预后的指标。FHIT基因、p21waf1/cip1基因在膀胱移行细胞癌的发生、进展中可能没有协同性。 Objective To investigate the protein expression of FHIT and p21^wafl/cip1 in normal bladder and bladder transitional cell cancer and the correlation between the two genes and bladder transitional cell cancer. Methods Immunohistochemical method was used in detecting the expressions of FHIT and p21^wafl/cip1 in 43 BTCC and 14 normal bladder tissues. Results The expression of Fhit protein in BTCC was lower than in nomal bladder tissues ( P 〈 0.05 ). There was no significant correlation between the expression of Fhit protein and pathological grade and clinical stage of BTCC( P 〉 0.05). The expression of Fhit protein in GI and superficial tumor was lower than in nomal bladder tissues( P 〈 0.05). There was no significant difference between the expression of Fhit protein in incipient tumor and in recurring tumor( P 〉 0.05). The expression of p21^wan/cipl protein in BTCC was lower than in nomal bladder tissues( P 〈 0.05). There was significant correlation between the expression of p21^wan/cipl protein and pathological grade and clinical stage of BTCC ( P 〈 0.05 ). There was no significant difference between the expression of p21^wan/cipl protein in GI and in nomal bladder tissues( P 〉 0.05). There was significant difference between the expression of p21^wan/cipl protein in incipient tmnor and in recurring tmnor ( P 〈 0.05). NO significant correlation was found between FH1T and p21^wan/cipl ( P 〉 0.05 ). Conclusions FH1T may be a useful tmnor marker in earl), diagnosis of bladder transitional cell carcinoma, p21^wan/cipl may be a useful tumor marker in judging the invasion and degree of malignanee and prognosis of bladder transitional cell carcinoma. There is no correlation between FH1T and p21^wan/cipl in the carcinogenesis of bladder transitional cell carcinoma.
