结直肠癌患者合并皮肤表现的临床分析

Cutaneous manifestations of colorectal carcinoma

目的探讨结直肠癌患者合并皮肤表现的临床特点。方法回顾分析8例伴有皮肤表现的结直肠癌患者的诊疗及预后资料。结果8例患者中,7例为结直肠癌术后发现皮肤或皮下肿物,1例为诊断直肠癌时合并副肿瘤性天疱疮,皮肤活检示棘细胞间和基底膜有IgG和C3沉积,病理证实为腺癌皮肤转移及副肿瘤性皮肤病。4例行术后化疗,1例合并副肿瘤天疱疮者予激素及免疫抑制剂治疗后皮损有缓解。本组患者诊断距皮肤改变时间为1～15个月,生存3～21个月,均死于肿瘤广泛转移。结论结直肠癌皮肤表现临床罕见,诊断时患者已经出现广泛转移且预后不良。

Objective The purpose of this study is to investigate the clinical features of cutaneous manifestations of eoloreetal eareinoma. Methods We retrospeetively analyzed eutaneous manifestations of 8 eases of eoloreetal eareinoma in our hospital from January 1996 to Deeember 2006. Results Seven eases were admitted with symptoms of eutaneous mass/or subeutaneous mass after operation. One ease was presented with paraneoplastie pemphigus when diagnosed as reetal eareinoma, The diagnosis was made pathologieally by revealing the origin of eutaneous manifestations. Conclusions Cutaneous manifestation is very rare and is typieally suggestive for widespread disease with poor prognosis.
