眼球钝挫伤眼底荧光血管造影分析

Analysis of fundus fluorescence angiography of eyeball obtuse bruise

目的眼球钝挫伤眼底荧光血管造影的分析。方法对38例(48只眼)眼球钝挫伤行眼底荧光血管造影。结果视神经挫伤(8只眼),视盘水肿、出血、FFA示视盘毛细血管扩张及渗漏晚期呈强荧光;视网膜震荡(22只眼),眼底表现为后极部视网膜水肿,黄斑中心凹反射消失。FFA:视网膜静脉早期后极部视网膜可见数片遮蔽荧光,小静脉管壁不均匀轻度荧光着染;视网膜出血(8只眼),FFA表现出血区荧光遮蔽;脉络膜裂伤(7只眼),视盘颞侧可见一条或数条黄白色弧形或条纹样,凹面朝向视盘。FFA:血管下方弱荧光造影,晚期呈高荧光;黄斑裂孔(3只眼):其中板层裂孔1只眼,FFA未见异常荧光;全层裂孔(2只眼),FFA显示为黄斑区圆形透见荧光。结论眼底荧光血管造影能及时准确了解眼球钝挫伤的部位和程度,同时结合病史、临床表现,综合分析做出准确诊断。

Objective：To analyze the fundus fluorescence angiography（FFA） of eyeball obtuse bruise.Methods： The fundus fluorescence angiography was made in 38 cases of eyeball obtuse bruise（48 eyes）.Results： FFA indicated laser disk dropsy,bleeding,the strong fluorescence in laser disk blood capillary expansion and leakage of later period in optic nerve bruise（8 eyes）.Retina dropsy in posterior polar and disappearance of concave reflection of central fovea of macula were shown in the eyeground in retinal concussion（22 eyes）.FFA indicated several pieces of camouflage fluorescence in the posterior polar retina of the early stage retina vein and uneven light fluorescence dying on the vessel wall of the small vein.FFA indicated fluorescence camouflage in the area of retina bleeding in 8 eyes while in choroid lacerated wound,one or several arcs or stripes of yellow and white with the concave side toward the laser disk were seen in the side of the temporal bone of laser disk.Weak fundus fluorescence angiography of underneath blood vessel became strong in advanced stage.Abnormal fluorescence was not seen in macula hole（of 3 eyes） and lamina hole（1 eye）.FFA indicated circular fluorescence in full level macula hole（2 eyes）.Conclusion： Fundus fluorescence angiography can accurately indicate the location and severity of eyeball obtuse bruise and accurate diagnosis can be made by the comprehensive analysis in combination with case history and clinical symptoms.
