摘要 目的探讨磁共振体层血管造影(megntic resonance tomographic angiography,MRTA)对面肌痉挛的诊断价值。方法回顾性地分析60例经手术治疗的面肌痉挛患者的MRTA表现及手术结果。结果60例症状侧MR-TA检查有血管压迫者为41侧,接触者为10侧,共51侧,敏感性为85%。60例症状侧行手术治疗,手术发现有血管压迫或接触者48侧,MRTA检查诊断神经血管压迫和接触的特异性分别为95.12%和80%。结论MRTA成像作为一项新的影像学检查技术,能较好地显示面神经脑池段与周围血管之间的关系,从而为面肌痉挛的病因诊断及临床上面神经微血管减压术(MVD)术前评估提供可靠的影像学依据。 Objective:To evaluate diagnostic value of magnetic resonance tomographic angiography(MRTA) in hemifacial spasm.Methods: MRTA manifestations of 60 patients with hemifacial spasm were reviewed retrospectively and operation results were compared.Results: 41 sides had vascular compression and 10 sides had vascular contact in 60 patients with one-side hemifacial spasm by MRTA,with the total of 51 sides,the sensitivity was 85%.48 sides with vascular compression or contact were found by the operation in 60 patients,the specificity of MRTA diagnosis of neurovascular compression and contact was 95.12% and 80%.Conclusin: MRTA as a new imaging technique can demonstrate the relationship between the cisternal portion of facial nerve and surrounding vascular structures,and provides reliable imaging evidence for pathogenetic diagnosis and preoperative examination for MVD.
