围手术期抗菌药物使用情况的调查与分析

Investigation and analysis of antibiotics applied during perioperative period

目的调查和分析医院围手术期抗菌药物使用情况。方法回顾性调查分析某医院2005年1月至12月600例围手术期患者抗菌药物使用情况。结果600例病例中,有532例病例使用了抗菌药物,其中,预防性用药443例,治疗性用药89例,抗菌药物使用率88.8%;两联以上使用的为294例,占55.2%;抗菌药物使用合理36例,占6.8%;不合理使用抗菌药物496例,占93.2%。手术预防用药时间统计为0-27 d,其中用药时间≥4 d的有299例,占67.5%。抗菌药物使用种类依次为头孢菌素类三代、硝基咪唑类、青霉素类＋β-内酰胺酶抑制剂、青霉素类、氟喹诺酮类等。结论围手术期抗菌药物选择不合理,围手术期用药时间、用药途径、用法不当,联合用药不合理,用药时间持续过长等较为普遍,围手术期预防性用抗菌药物应适当控制。

Objective To investigate and analyze the application of prophylactic antibiotics in perioperative period in hospital. Methods Totally 600 cases perioperative period were retrospective investigated from January to December in 2005. Results Among 600 cases, antibiotics was used in 532 cases（ 88.8% ）, including prevention usage 443 cases, remedial usage 89 cases; 294 cases（ 55.2% ） used two or more antibiotics; 36 cases（6.8% ） used rationally, 496 cases（93.2% ） used irrationally. The time of prevention usage was （0 -27） d, 229 cases（67.5% ） used more 4 d. The antibiotics included cephalosporins, nitroimidazole, penicillins accompany 13-1actamase enzyme inhibitor, penicillins and FQNS in order. Conclusion During perioperative period, application of antibiotics are irrationally in many cases, includes the medication time, application methods and association, it should be regulate in prevention usage.
