早期针灸治疗对重型颅脑损伤患者肢体运动功能的影响

Effects of early acupuncture on motor function of the limb in the severe head injury patients

目的:观察早期针灸介入对重型颅脑损伤患者肢体运动功能的影响。方法:将96例患者随机分为观察组(48例)和对照组(48例)。对照组采用神经外科手术和西药(甘露醇、胞二磷胆碱、头孢他啶等)常规治疗,观察组在对照组治疗的基础上,术后48小时～1周、病情稳定以后,在痰病的不同阶段采用不同的穴位针刺治疗,主穴取内关、曲池、外关、合谷、臂臑、肩髃、风市、血海、阴陵泉、三阴交、太冲等。采用简式Fugl-Meyer(FMA)积分评定,分别于治疗前和治疗10周后对肢体运动功能进行评定。结果:同组治疗前后比较,FMA评分均提高,经统计学处理,差异有非常显著性意义(P<0.01);治疗后2组间比较,观察组FMA评分提高的程度显著高于对照组(P<0.05, P<0.01)。结论:早期针灸可提高重型颅脑损伤患者肢体运动功能恢复的程度。

Objective To observe the effects of early acupuncture on motor function of the limb in the severe head injury patients. Methods Ninety-six patients were randomly divided into an observation group and a control group, 48 cases in each group. The patient in the control group were treated by routine treatment of neurosurgical operation and drug treatment （Mannitol, Citioline Sodium, Cefeazidime, etc. ）. While the patient in the observation group were treated by the routine treatment and acupuncture treatment 48 b-1 week after operation in different stages with different acupoints selected when the situation was stable. The selected main points were Neiguan （PC 6）, Quchi （LI 11）, Waiguan （TE 5）, Hegu （ LI 4）, Binao （LI 14）, Jianyu （LI 15）, Fengshi （GB 31）, Xuehai （SP 10）, Yinlingquan （SP 9）, Sanyinjiao （SP 6）, Taichong （LR 3）, etc.. The limb motor function was evaluated with FuglMeyer Assessment （FMA） scale before treatment and after 10-week treatment. Results There was a significant improvement in the FMA score in the two groups after the treatment （P〈0.01）. The comparison between the two groups after the treatment showed that the improvement of FMA score in the observation group was significantly better than the control group （P〈0.05 or P〈0. 01）. Conclusion Early acupuncture can improve the limb motor function in the severe head injury patients.
