65例青春期卵巢肿瘤临床及病理分析

A clinical and pathologic analysis on 65 cases of ovarian tumors in adolescents

目的探讨青春期卵巢肿瘤的临床及病理特点。方法回顾性分析我科65例10～19岁卵巢肿瘤患者相关资料。结果临床表现主要为腹痛、腹胀、下腹部肿物;病理类型:生殖细胞肿瘤31例(恶性3例),上皮性肿瘤23例(交界性2例,恶性1例),性索间质细胞肿瘤1例,其他10例。治疗方法:良性肿瘤均经开腹或腹腔镜行一侧附件切除术或卵巢囊肿剔除术,交界性肿瘤行患侧附件切除,恶性肿瘤患者术后辅以化疗。结论青春期少女卵巢肿瘤以生殖细胞肿瘤及上皮性肿瘤为多见,治疗应兼顾治愈肿瘤和保留生育功能,治疗方案应个体化。

Objective To analyze the clinical and pathologic characteristics of ovarian neoplasms in adolescents. Methods A retrospective analysis was performed on 65 patients of ovarian tumors between the age of 10 and 19 during the period of 2004 to 2007 in the First Affiliated Hospital of Nanjing Medical University. Results The majority of patients presented with abdominal pain, abdominal distention and a pelvic mass. The pathologic types included germ cell tumors in 31 cases（3 malignant）, epithelial ovarian tumor in 23 cases （2 borderline and 1 malignant）, sex-cord stromal tumor in 1 case and 10 other types. An abdominal unilateral salpingo-oophorectomy or a laparoscopic cystectomy was performed on those with benign tumors. In patients with borderline tumors, a unilateral salpingo-oophorectomy was undertaken. A chemotherapy was given to the patients with malignant tumors after surgical treatment. Conclusion The germ cell tumors and epithelial ovarian tumors were more common. In the patients with malignant tumors, the therapeutic strategy should be individualized, and both cure and preservation of fertility should be considered.
