代谢综合征三种工作定义在广州地区人群中的应用比较

Study on the comparison of three diagnostic criteria of metabolic syndrome in Guangzhou population

目的比较国际糖尿病联盟(IDF2005)、美国国家胆固醇教育计划成人治疗组第三次指南(ATPⅢ2001)及中华糖尿病学会(CDS2004)提出的代谢综合征(MS)工作定义在广州地区人群中的应用。方法采用广州地区居民糖尿病流行现况及其防治模式探讨的调查资料,对具有MS组分测量值的社区人群(6709人),分别采用IDF(2005)、ATPⅢ(2001)和CDS(2004)定义诊断MS,并比较MS的患病率及诊断的一致性。结果应用IDF(2005)、ATPⅢ(2001)和CDS(2004)定义,诊断出的MS患病率分别为18.6%、17.2%和10.2%。ATPⅢ(2001)与CDS(2004)、IDF(2005)与CDS(2004)和IDF(2005)与ATPⅢ(2001)诊断一致率分别为89.9%、86.4%和86.9%,Kappa值分别为0.58、0.46、0.55。结论ATPⅢ(2001)与CDS(2004)定义、IDF(2005)/CDS(2004)及IDF(2005)/ATPⅢ(2001)代谢综合征诊断一致率基本相同,均为中度联系。

Objective To compare the application in Guangzhou population of the definition for metabolic syndrome （MS） proposed by the IDF in 2005, ATPⅢ in 2001 and CDS in 2004. Methods Based on the population in Guangzhou from an epidemiologlcal study on diabetes mellitus and its prevention and control model, the study was conducted including 6709 cases, with data of plasma glucose, lipid profile, blood pressure, etc. MS was diagnosed and compared according to those 3 definitions respectively. Results According to the IDF（2005）, A.TPⅢ （2001）, and CDS（2004）, the crude prevalence rate of MS was 18.6%, 17.2% and 10.2% respectively. The agreement rate in the diagnosis of MS between using ATPⅢ （2005） and CDS（2004） definitions was 89.9 %, the agreement rate in the diagnosis of MS between using IDF（2005） and CDS definitions was 86.4 %, and the agreement rate in the diagnosis of MS between using IDF（2005） and ATPⅢ （2001） definitions was 86.9%, its value of Kappa being 0.58, 0.46, 0.55 respectively. Conclusions The agreement in the diagnosis of MS using the ATPⅢ （2001） and CDS（2004） definition is the same as using IDF（2005） and another two definitions, and belongs to medium association.
