大剂量硝普钠治疗充血性心力衰竭伴低血压的疗效观察

Obersavition of Effect of High Doses of Sodium Nitroprusside on Congestive Heart Failure Accompanied with Hypotension

目的评价大剂量硝普钠治疗充血性心力衰竭伴低血压患者的疗效和安全性。方法68例充血性心力衰竭伴低血压患者随机分为小剂量硝普钠治疗组(A组)和大剂量硝普钠治疗组(B组),连续应用7 d。治疗前后分别进行心率、心脏超声和血压检查。结果治疗后B组患者的左室舒张末期内径缩小率、左室收缩末期内径缩小率、左室射血分数改善率均比A组患者显著。两组患者心率、收缩压和舒张压没有明显变化。结论大剂量硝普钠治疗充血性心力衰竭伴低血压患者是安全的,能明显改善左室心功能。

Objective To evaluate the efficacy and safety of high doses of sodium nitroprusside in the treatment of congestive heart failure accompanied with hypotension. Methods 68 cases of congestive heart failure accompanied with hypotension were randomly divided into treatment with either low doses （group A） and high doses of sodium Nitroprusside （group B ）. 7 days after treatment in succesion, heart rate, echocardiography and blood pressure were performed. Results The change of left ventricular end-diastolic diameter, left ventricular end-systollc diameter and left ventrlcular eject fraction in group B was greater than that in group A. There was no apparent difference in heart rate, systolic blood pressure, diastolic blood pressure before and after treatment in both groups. Conclusion High doses of sodium nitroprusside are safe in the treatment of congestive heart failure accompanied with hypotension, and have a favourable effect on left ventricular.
