机械通气相关性肺炎60例临床分析

Analysis of 60 Patients with Mechanical Ventilator-associated Pneumonia

目的探讨机械通气相关性肺炎易患因素及防治措施。方法对109例机械通气患者中60例发生机械通气相关性肺炎患者进行回顾性分析。结果60例患者治愈18例,好转出院24例,死亡18例(30.0%)。结论年老、机械通气时间长、有基础疾病、营养状况差发生率高,早期病原学诊断、敏感抗生素应用是有效治疗的关键措施,对多种抗生素耐药是造成死亡的主要原因。

Objective To explore and discuss some important issues of mechanical ventilator-associated pneumonia, the possible occasions and the means for prevention and cure. Methods Among 109 mechanically ventilated patients, 60 patients with mechanical ventilator-associated pneumonia, have been reviewed and analyzed. Results Among 60 patients, 18 patients were completely recovered; 24 patients were improved and discharged; and 18 patients, 30% of the 60 cases,were dead. Conclusion There is high occurrence frequency in the people at old age, or long time in mechanical ventilator, or having some fundamental diseases, or at poor nutrition status ; and that the pathogenic diagnosis at early stage and the application of the sensitive antibiotics are the key means for the effective cure. Moreover, it is deduced that the resistance to several kinds of antibiotic is the major cause of death.
