X线指引下平卧位剑突下心包穿刺置管引流术的应用

Application of X-ray to guide percutaneous pericardial drainage through xiphoid with catheter while adopting decubitus in the management of pericardial effusion

目的评价X线指引下平卧位剑突下心包穿刺置管引流术的方法及安全性。方法X线指引下平卧位剑突下心包穿刺置管引流术方法。结果35例患者均一次穿刺成功,无穿刺相关并发症出现,首次抽液量(350±170)mL,总引流量(580±220)mL;猪尾导管留置时间(9.5±7.7)d。留置导管过程中无一例发生心包继发感染。结论X线指引下平卧位剑突下心包穿刺置管引流术是安全有效的,值得推广。

Objective To evaluate the technique and safety of application of X-ray to guide percutaneous pericardial drainage through xiphoid with catheter while adopting decubitus in the management of pericardial effusion. Methods 35 patients with moderate or large pericardial effusion were percutaneously puncture under the guide of X-ray for continuous pericardial drainage through xiphoid with catheter while adopting decubitus. Record the first and the cumulative drainage volume. Observe the complication of this technique and the time of remaining catheter. Results thirty-five patients of pericardial effusion were successfully punctured and remained catheter without complication. The first drainage volume was （350±170） mL and the cumulative drainage volume was （580±220） mL. The average drainage time were （9.5±7.7） d. There were no patient suffered secondary pericardium infection. Conclusion The use of X-ray to guide percutaneous pericardial drainage with catheter through xiphoid while adopting decubitus is a effective, safe way. It also can take the place of traditional methods.
