磁共振动态增强扫描在垂体微腺瘤诊断中的价值 被引量:9

摘要 目的:评估MRI动态增强扫描的检查质量和其在垂体微腺瘤诊断中的价值。方法:分析经临床及实验室检查诊断为垂体微腺瘤38例的MRI平扫、钆喷替酸葡甲胺动态增强扫描及常规延迟增强扫描图像,判断钆喷替酸葡甲胺动态增强扫描图像质量,以及统计MRI平扫、钆喷替酸葡甲胺动态增强扫描及常规延迟增强扫描检出率。结果:MRI平扫、动态增强扫描及常规延迟增强扫描对病灶检出分别16例(42.1%)、34例(89.5%)、26例(68.4%)。3种不同的扫描方法病灶检出率差异有统计学意义(P<0.05)。结论:MRI钆喷替酸葡甲胺动态增强扫描是相对简单、易行、成功率高的检查方法,可明显提高垂体微腺瘤的检出率,可作为MRI诊断垂体微腺瘤的常规检查方法。 Objective :To evaluate the examinational quality of dynamic contrast enhancement MRI and its effect in diagnosing pituitary microadenoma. Methods :The data of 38 cases with pituitary microadenoma confirmed by clinical manifestation and biochemical tests were studied by plain MRI, Gd-DTPA dynamic enhanced MRI and conventional delayed enhanced MRI. To evaluate the quality of image of Gd-DTPA dynamic enhanced MRI ,and statistics the rate of lesion detection of plain MRI, Gd-DTPA dynamic enhanced MRI and conventional delayed enhanced MRI. Results :The positive rate in Gd-DTPA dynamic enhanced MRI was 100%, the positive rate of diagnosis for pituitary micro adenoma with plain MRI scan, dynamic enhanced MRI and conventional delayed enhanced MRI was 42.1%(16/38), 89.5%(34/38) and 68.4%(26/38) respectively, and there were statisticaly significances(P〈0.05 )among different scanning methods. Conclusion:Gd-DTPA dynamic enhanced MRI was relatively simple, easy, efficientand obviously improved its detection rate. Dynamic enhanced MRI should be considered as the conventional examinational method in diagnosing pituitary microadenoma.
