动力型外固定架治疗老年桡骨远端粉碎骨折

Dynamic External Fixation in the Treatment of Comminuted Fractures of the Distal Radius Bone

目的探讨使用动力型外固定架治疗老年桡骨远端粉碎性骨折的临床疗效。方法自2004年1月开始选择性地采用动力型外固定架治疗老年桡骨远端粉碎性骨折共22例23侧。其中男9例,女13例,年龄51~85岁,平均60.8岁。按AO/ASIF分型,均为C型骨折,其中C1型6侧,C2型8侧,C3型9侧。按受伤机制分:摔伤16例,高处坠落伤6例7侧肢体;开放性骨折5例,皮肤与软组织损伤较轻。受伤到治疗时间为2 h^13 d,平均4.3 d。结果术后随访6~20个月,平均12个月。解剖复位15例16侧肢体,功能复位7例;关节功能按Dienst功能评估标准进行评定,优15例16侧肢体,良4例,可2例,差1例,优良率为87.0%。无神经血管损伤、伤口感染及骨髓炎等并发症。结论采用动力型外固定架是治疗老年桡骨远端粉碎性骨折的一种较好的方法,其操作简单、固定可靠、疗效满意、并发症少。

Objective To explore the use of dynamic external fixator for the treatment of age-related clinical efficacy of the distal radius fractures. Methods From January 2004 to December 2006 using dynamic external fixator for the treatment of total of 22 cases of senile distal radius fractures 23 limbs. 9 males, 13 females ,aged 51 485 years, with an average age of 60.8. AO/ASIF classification are ：C fractures,Cl-six cases ,eight cases of C2 and C3-nine cases. ： 16 cases were injured by a mechanism of fall ,six cases 7 limbs of high fall ;five cases of open fractures ,lighter skin and soft tissue injury. Treatment for injuries from two hours to 13 days,with an average of 4.3 days. Results 6420 months follow-up ,an average of 12 months. Joint function was assessed by function evaluation criteria ：excellent in 15 cases （16 limbs）, good in four cases,and poor in 1 case;excellent rate was 87.0%. Surgical treatment without neurovascular injury, no wound infections such as osteomyelitis. Conclusion Using dynamic external fixator for the treatment of distal radius fractures in the elderly is a better way;simple to operate,reliable fixation and the results were satisfactory.
