扩大肾窦治疗复杂肾结石

Operation by enlarged renal sinus to treat complicated kidney stone

目的提高复杂肾结石的手术疗效,降低手术难度。方法回顾分析162例经扩大肾窦切开取石的复杂肾结石患者的临床资料。结果所有患者顺利完成手术,术中不阻断肾血流,手术时间90～245min,平均139min,术中出血量110～480mL。术后3月复查腹部平片示有24例有结石残留,7例经体外振波碎石,4例经肾造瘘管经皮肾镜下碎石,13例泥沙样小结石自动排石。12月复查静脉肾盂造影示肾显影良好,积水明显减少,未见结石复发和肾盂狭窄。结论该术式操作简单,出血少,易取石,对肾功能影响小,是治疗复杂肾结石理想的手术方法之一。

[Objective] To improve operation curative effect of complicated kidney stone and degrade operation difficulty. [Methods] We retrospectively analyzed clinical data of 162 patients with complicated kidney stone who accepted operation by enlarged renal sinus. [Results] All operations were successfully completed and none blockade renal blood flow. Operation time was 90 min to 245 min, average 139 min. Blood loss volume were 110 mL to 480 mL 24 patients had residual calculus when recheck KUB, 7 accepted ESWL, 4 PCN, and 13 automatic removing urinary calculus. IVU showed that renal visualization was well, hydrops obviously decreased, none calculus recured or renal pelvis stegnosis. [Conclusion] Operation by enlarged renal sinus is simple, less blood loss, easily calculus removed, littlely effect renal function, and ideal approach treatment complicated kidney stone.
