腹腔镜辅助下左肝外叶切除治疗左肝胆管结石

Laparoscopy-assisted left lateral liver resection for hepatolithiasis

目的总结腹腔镜辅助下左肝外叶切除治疗胆管结石的临床经验。方法自2002年6月至2005年10月期间，本院左肝内胆管结石患者21例。患者在腹腔镜辅助下行左肝外叶部分切除及胆道镜探查取石术，部分加做胆囊切除术。结果全部21例行腹腔镜辅助下左肝外叶切除术，17例联合行腹腔镜胆囊切除术。手术时间为90—190min，平均（140±36）min。手术中出血量为100—250ml，平均（160±52）ml。术后恢复顺利，术后平均住院（6．2±1．2）d。结论腹腔镜辅助下行左肝外叶部分切除治疗左肝胆管结石是安全可行的。该术式容易掌握，风险小，为治疗左肝胆管结石提供了一个新方法。

Objective To summarize the clinical experience of laparoscopy - assisted left lateral liver resection, of hepatolithias＇s. Methods From June 2002 to October 2005, laparoscopy-assisted left lateral liver resections were performed in 21 patients with left intrahepatic stone, and some patients underwent laparoscopic cholecystectomy additionally. Results All the procedures were successful. Operation time was 90 - 190 min, average（ 140 ± 36 ） min. The blood loss in operation was 100 - 250 ml, average （ 160 ±52 ） ml. No complication occurred. The average postoperative hospital stay was （ 6.2 ±1.2 ） days. Conclusion Laparoscopy-assisted left lateral liver resections of hepatolithiasis are safe and minimally invasive, which provides a new method for hepatolithiasis.
