摘要
目的探讨尿β2微球蛋白(β2-MG)和白蛋白(Alb)在窒息新生儿早期肾损害临床诊断中的价值。方法对42例窒息新生儿(重度窒息组18例,轻度窒息组24例)和22例正常新生儿生后24小时内采用放射免疫法分别测定尿β2-MG和Alb及血清尿素氮(BUN)、肌酐(Cr)水平,用以比较。结果生后24小时内,重度窒息组尿β2-MG和Alb明显高于轻度窒息组(P<0.05),轻度窒息组尿β2-MG和Alb高于正常儿组(P<0.05),此时3组血清BUN和Cr并无明显差异(P(0.05)。结论生后24小时内测定尿β2-MG及Aib早期评价新生儿窒息肾功能损害,判断肾小管损害具有重要的意义。
Objective To probe into clinical and diagnostic value of β2 -microglobin (β2 -MG) and albumin(Alb) levels in urine in early renal damage of neonatal asphyxia. Methods The Radio - immunity method was used to measure β2 - MG and Alb levels in urine as well as Urea -nitrogen of Blood(BUN) and creatinine (Cr) in serum for the 42 neonatal asphyxia (18 cases in severe asphyxia group and 24 cases in mild asphyxia group) and 22 normal neonatal cases within 24 hours after their birth. Results In the first day of their birth for the two group neonatal asphyxia the levels of β2 - MG and Alb in urine of severe group were higher than those of the mild group ( P 〈 0.05 ), the levels of β2 - MG and Alb in urine of mild group were higher than those of the normal group while there was no significant differences of blood urea - nitrogen and serum creatinine between the three groups ( P 〈 0.05 ). Conclusion Theβ2 - MG and Alb test in urine is of great significance in evaluation of early renal functional damage, especially for the judgment of injury of renal tubule in neonatal asphyxia.
