摘要
目的了解免疫固定电泳(IFE)对单克隆免疫增殖性疾病诊断的价值。方法采用琼脂糖电泳+免疫固定技术;免疫球蛋白、血、尿轻链采用免疫散射比浊法测定。结果1184例患者血清蛋白电泳有31例检出M蛋白,检出率为2.62%,其中恶性单克隆丙球蛋白血症患者27例,血清经免疫固定电泳均出现致密深染单克隆M蛋白,以IgGκ型为主占33.3%。良性单克隆丙球蛋白血症(MGUS)4例,占0.34%。血清经蛋白电泳均出现染色较浅单克隆M蛋白。经免疫固定电泳,结合Ig定量检测并依据判断标准确认为良性单克隆丙球蛋白血症〗。结论对单克隆免疫增殖病的良性和恶性的鉴别,IFE是必不可少的检测手段。MGUS患者应定期检查,以便及早发现,及时治疗。
Objective To investingate the diagnostic value of immunofixation electrophoresis (IFE) in monoclonal gammopathy of uncertain significance (MGUS). Methods Agarose gel electrophoresis and immnofixation was used in the study. Immunoglobulin and light chain in serum and urine were detected by using immunoturbidimetry. Results 31 patients (2.62%) were found M proteins in a total 1184 of patients. 27 patients of 31 patients were found malignancy monoclonal gammapathy with a high density of monoclonal M proteins in sera by using IFE and all were IgG kappa chain. In addition, 4 patients (0.34%) were found MGUS with a low density of M proteins in sera. Conclusion IFE is a key method for detection and identification of benign and malignancy immunoproliferation disease and the patients with MGUS should examine regularly and treat early.
