摘要
目的:研究核素唾液腺显像定量分析对诊断干燥综合征的临床价值。方法:对30例原发性干燥综合征(A组)和32例继发性干燥综合征(B组)患者的唾液腺核素动态显像资料进行分析,并与20例甲状腺结节(C组)进行对照。结果:A、B组唾液腺功能的正常率与C组比较差异有统计学意义(均为P<0.05),A、B组唾液腺功能的正常率、轻度受损率及重度受损率比较差异无统计学意义(均为P>0.05)。在定量分析中,A、B组双侧腮腺腺体摄取指数明显低于C组(均为P<0.05)。A、B组双侧腮腺及左侧颌下腺的腺体排泄率显著低于C组(均为P<0.05)。3组的腺体相对摄取率比较差异均无统计学意义(均为P>0.05)。结论:核素唾液腺显像定量分析可客观评价干燥综合征患者唾液腺功能的受损状况,其中以双侧腮腺腺体摄取指数和腺体排泄率较为可靠。
Objective: To study the clinical value of quantitative analysis of salivary gland seintigraphy in the diagnosis of Sjoegren syndrome(SS). Methods: Salivary gland scintigraphy data of 30 patients with primary SS (group A) and 32 patients with secondary SS (group B) were quantitatively analyzed. Data of 20 patients with thyroid nodules were used as control group (Group C). Results: The normal rates of salivary gland function in group A and group B were significantly lower than that of group C (P 〈0.05 ). There were no significant differences in the rate of normal salivary gland function, slight damage and severe damage between group A and group B ( P〉0.05 ). In quantitative analysis, the uptake index (UI) of both parotid glands was significantly lower in group A and group B than that of group C (P〈0.05). The excretion fractions (EF) of both parotid glands and the left submandibular gland of group A and B were significantly lower than that of group C ( P 〈0. 05). Only, the relative uptake ratio (rUR) of salivary gland was no different in all groups ( P〉0. 05 ). Conclusion : Quantitative analysis of salivary gland scintigraphy can objectively estimate the degree of salivary gland function damage in patients with SS, with UI and EF being more reliable parameters.
