Clinical Application of Minitraumatic Dynamic Hip Screw with Fluoro-navigation for the Femoral Intertrochanteric Fracture in Aged Patients

摘要 目的探讨在X线透视导航下微创置入动力髋螺钉(DHS)治疗老年股骨转子间骨折的临床应用。方法自2005年12月~2006年6月,笔者应用导航下微创方法置入DHS治疗老年股骨转子间骨折18例,男5例,女13例;年龄60~92岁,平均76岁。结果术后随访5~12个月,平均9个月。18例股骨转子间骨折的应用结果令人满意。手术时间平均为105min。X线图像采集平均5次。术中和术后无并发症发生。结论使用导航系统微创置入DHS不仅进针方向精确、拉力螺钉位置好、切口小,而且还可以减少术者的X线暴露时间,是治疗老年股骨转子间骨折的理想方法。 Objective To explore clinical application of fluoro - navigation in minitraumatic dynamic hip screw (DHS) fixation for the femoral intertrochanteric fractures in aged patients. Methods From December 200.5 to June 2006, 18 aged patients with intertrochanteric fracture (5 males and 13 females) were treated with minitraumatic dynamic hip screw in fluoro- navigation. Their ages ranged from 60 to 92 years, 76 year old on average. Results The patients were followed up for 5 to 12 months, 9 months on the average. The results of fixation for 18 femoral intertrochanteric fractures were satisfactory. The average duration of surgery was 105 minutes, and the average time of X - ray screening was 5. No complication was recorded during surgery and after surgery. Conclusion Fluoro - navigation can facilitate DHS as a minimally invasive surgery because of its accurate guidance to nail insertion and lag screw positioning, a smaller surgical wound and minimized X - ray exposure suffered by operation theater staff. It proves to be an ideal device to treat intertrochanteric fracture in aged patients.
