十二指肠降部溃疡的临床特点与钡餐检查X线表现

Clinical characteristics and X-ray findings of the ulcer in duodenal descending part

目的探讨十二指肠降部溃疡的临床表现及X线特点。方法经胃镜和手术证实的17例十二指肠降部溃疡的患者,均经低张气钡双重造影术。结果黏膜纠集现象6例,溃疡部肠管狭窄14例,溃疡部位以上肠管扩张13例,其中,呈"c"型扩张7例,"s"型扩张6例,十二指肠球部扩大14例。结论十二指肠降部溃疡临床诊断易漏诊或误诊,结合临床特点和钡餐多轴位加压透视检查,可以提高诊断率,为临床治疗提供依据。

Objective To discuss the clinical characteristics and X -ray findings of the ulcer generated in duodenal descending part. Methods Seventeen patients with gastroscopically and surgically proved the ulcer in duodenal descending part underwent gastrointestinal hypotonic and air - barium double contrast examinations. Results Among 17 cases, mucosal gathering phenomena were found in 6, intestinal tube narrowing of the ulcer segments in 14, intestinal tube dilatation over the ulcer segments in 13 which comprised 7 c - type dilatation and 6 s - type dilatation, and enlargement of duodenal bulb parts in 14. Conclusion Clinically, the ulcer of duodenal descending part is easily leak - diagnosed or misdiaguosed. The integrative analysis of clinical characteristics and barium meal test plus polyaxial pressure fluoroscopy can elevate the diagnostic accuracy of this disease and can provide the evidence for guiding clinical treatment.
