高贵金属PFM对48例前牙美容修复效果的临床观察

Clinical studies on aesthetically restorative efficacy of 48 cases of anterior teeth with porcelain fused to high noble metal alloy

目的：观察高贵金属烤瓷熔附金属全冠（porcelain fused to metal，PFM）对前牙美容修复的临床效果。方法：将48例以高贵金属PFM进行活髓前牙美容修复患者的196颗修复牙体作为实验组，将相同数目的正常邻牙作为对照组，于修复后2月、1年、3年、5年对随诊病例进行临床观察，检查其修复效果。结果：196颗修复体于修复后2月、1年、3年、5年基牙均无继发龋、牙髓炎以及颈缘牙龈变色，患者对修复体舒适性均表示满意，瓷损坏发生率为0．52％。在修复后2月、1年、3年及5年各时段实验组的牙周探诊深度及龈沟出血指数均高于对照组，存在显著性差异（P〈0．05），说明PFM对实验组牙周组织有一定影响，而实验组在2月～5年不同时段之间的牙周探诊深度及龈沟出血指数均无显著性差异（P〉0．05）。结论：高贵金属PFM对前牙美容修复临床效果良好，虽对牙周组织存在一定不利影响，但并不会随着时间的延长而逐渐加重。

Aim： To observe clinical aesthetically restorative efficacy of anterior teeth with porcelain fused to high noble metal alloy. Methods： With 196 restored teeth of 48 patients as trial group and normal adjacent teeth in same patients as control. To make clinical examinations for restorative efficacy in 2months, lyear, 3years, 5years. Results： There were no secondary caries, pulpitis and color change of gum on abutments of 196 restorations. All patients were satisfied with comfort of restorations. The incidence rate of porcelain damage was 0.52%. Periodontal deep and sulcus bleeding index showed evident difference between trial group and controlled group showed no difference at different times from 2months in 2months, lyear, 3years, 5years（ P 〈 0.05 ） ; but to 5years（ P 〉 0. 05 ）. Conclusion： Porcelain fused to high noble metal alloy can meet the requirement of cosmetic restoration with a good result in clinical application. And it has negative influences on gingival health of the restored teeth without positive correlation to time.
