不同剂量咪唑安定鞘内应用对腰麻效果的影响 被引量:1

摘要 【目的】以探讨不同剂量鞘内咪唑安定的临床价值。【方法】择期腰4~5或腰5~骶1椎间盘髓核摘除手术病人60例,手术前病人随机等分为4组:B组(布比卡因对照组)鞘内注入7.5g/L布比卡因2mL(15mg),腰麻;M1组(咪唑安定1组,以下类推)腰麻前鞘内注入力月西1.0mg;M2组腰麻前注入力月西1.5mg;M3组腰麻前注入力月西2.0mg。病人腰麻平卧后测试麻醉阻滞上升平面,术中观察麻醉效果及镇静情况,术后随访24h疼痛、镇痛用药、恢复情况及其他反应,围术期持续监测呼吸、循环状态。【结果】4组患者的一般情况相同(P>0.05),围术期呼吸、循环稳定顺利完成手术。镇静效果M3>M2=M1>B组,M3组效果最佳(P<0.05)。与B组对照,M2和M3组术后第一疼痛时间明显延长(P<0.05~0.01),M3组时间最长(P<0.05~0.01);术后肌注镇痛药曲马多百分比为B组86.7%、M1组73.3%、M2组66.7%、M3组33.3%,仅M3组用药最少(P<0.05)。麻醉效果评分优等B组20%、M1组33.3%、M2组53.3%、M3组86.7%,M3效果最佳(P<0.05)。【结论】鞘内咪唑安定2mg可明显改善腰麻的效能,无明显副作用。
