HIFU结合全身化疗治疗宫颈癌放疗后残留17例近期疗效观察

The application of high intensity focused uiltrasound(HIFU) combined with chemotherapy on 17 cases with rasidual cervical cancer after radiotherapy

目的探讨高强度超声聚焦热凝技术(HIFU)结合全身化疗治疗宫颈癌放疗后残留病灶的疗效。方法选择本院收治的放疗后残留或短期内复发进展的宫颈癌患者17例作为治疗对象。应用HIFU NIT 9000P型高强度超声聚焦肿瘤消融机实施脉冲式释放高强度超声治疗,同时辅以全身化疗。比较治疗前后患者的临床症状和肿瘤体积的变化。结果(1)肿瘤体积改变:完全缓解2例,部分缓解9例,稳定4例,总有效率88%;(2)疼痛分数改变:治疗前(4.53±1.12),治疗后(1.35±1.62),p<0.001,差异非常显著;(3)生活质量改变:KPS评分治疗前(67.65±5.62),治疗后(82.35±10.33),p<0.05,差异有显著性。结论HIFU技术配合全身化疗治疗放射治疗后残留或近期复发进展的宫颈癌,局部控制性好,近期有效率高,可作为宫颈癌治疗中与放疗、化疗结合综合应用。

Objective To evaluate the treatment of rasidual cervical cancer after radiotherapy by high intensity focused ultrasound （HIFU） combined with chemotherapy, Methods A total of 17 cases of rasidual or recurrence cervical cancer were treated by HIFU （NIT9000P TYPE）and chemotherapy, The efficacy was evaluated according to the change of tumor size and improvement of clinical symptoms. Results （1） The change of tumor size： two cases achieved complete remission, 9 cases portial remission, and 4 cases no change ,the tatal effective rate was 88% ;（2）The releice of pain score： the score was （4.53＋1,12） before treatment and （1.35＋l.62）after treatment with a significant difference （P〈0.001） （3）quality of life （KPS score ）： the KPS score was 67.65±5,62 before treatment , and 82.35±10.33 after treatment （P〈 0.05）. Condnsion The treatment of residual or recurrence cervical cancer after radiotherapy by HIFU combined with chemotherapy is effective for local control and has good efficiency.
