微创包皮整形术

The improved technology of prepuce circumcision

目的:探索一种包皮微创整形美容新术式。方法:根据美容整形原则,将微创技术用于临床,对包皮环切术方法进行改进。包括:①术前设计画线,确定内外板手术切除量;②用水剥离加钝、锐性分离方法,避免小动静脉损伤;③手术仅切除表皮,保留内外板间的筋膜、血管、神经;④缝合时,先于3、6、9、12点,定位缝合4针,提起相邻的两牵引线,采用由中间向两边平分的方法进行缝合;⑤在阴茎背部12点内板设计一三角形瓣,外板对应处纵形切开,将三角瓣插入其中。增加了包皮口的周径,术后切口变成曲线,不易形成瘢痕。结果:所有病例均无感染、出血、血肿,切口无瘢痕增生,均甲级愈合。结论:改进后的包皮环切术,创伤小,术后并发症少,术后包皮外观自然、功能好。

Objective To explore a improved technology for prepuce circumcision. Methods According to principles of plastic and aesthetic surgery,we designed a improved technology of prepuce circumcision： （1）Drawing lines of incision before operation to confirm excision quantity of inner and outer surface. （2）Avoiding injury of vessels by water-stripping and blunt dissection.（2）Exising skin only,maintaining nerve,vessel and fascia between inner and outer surface. （4）Forming a tri-angle flap at 12 o＇clock position of inner surface,incising vertically outer surface and inserting flap to increase the perimeter of prepuce. （5）Anchoring suture was performed at 3,6,9,12 o＇clock positions,adding interrupted suture to close the wound. Results No infection,hematoma and hyperplasia scar occurred in all patients. Conclusion The improved technology of prepuce circumcision has advantages with less injury and complication,and with natural appearance and good function of prepuce.
