16层螺旋CT冠状动脉显像和平板运动试验在诊断冠心病中的应用 被引量:3

摘要 目的通过与冠状动脉造影结果比较评价冠状动脉多层螺旋CT成像技术(CTA)和平板运动试验对冠心病的诊断价值。方法对72例疑似冠心病患者行冠脉造影(CAG)、16层螺旋CT冠状动脉显像(16-SCT)和平板运动试验(EET)。以冠脉狭窄≥50%作为CAG和16-SCT阳性结果,并对结果进行敏感性和特异性分析。结果以冠脉造影作为"金标准",16-SCT诊断冠心病的敏感性达到87.0%,特异性为83.3%。16-SCT和EET检查皆阳性者作为冠心病的诊断方法敏感性为53.7%,特异性为88.9%;将16-SCT、EET检查中一项阳性者为阳性组,敏感性达到90.7%,特异性为55.6%。结论16-SCT可以作为冠心病患者重要的筛选手段。联合平板运动试验可提高冠心病的检出率,可能减少假阳性率。 Objective To evaluate the value of 16-slice computed tomography (16-SCT) and exercise stress testing(EET) in the detection of coronary heart disease while the coronary angiography (CAG) was used as the golden standard.Methods 72 patients suspected to coronary heart disease (CHD) were underwent 16-SCT,EET and CAG.The specificity and sensitivity of those methods were observed. Results According to CAG, the sensitivity and specificity of 16-SCT were 87.0% and 83.3% respectively.If the patients whose results of 16-SCT and EET were both positive were diagnosed CHD,the specificity could be improved to 88.9%.If the patients whose results of 16-SCT or EET was positive were diagnosed CHD,the sensitivity could reached to 90.7%.Conclusion 16-SCT is a prespective method in diagnosing CHD. It can improve the incidence of CHD by combining EET.
