摘要 目的研究3种克雷伯菌所产各种β内酰胺酶(plase)分布情况。方法采用多底物纸片法(协同法、拮抗法)、超广谱G-内酰胺酶(ESBLS)确认试验、AmpC酶检测法检测51株克雷伯菌属[产酸克雷伯菌(K.ox)23株,肺炎克雷伯菌臭鼻亚种(K.oz)18株,土生克雷伯菌(K.te)10株]所产各种β—lase。结果(1)51株克雷伯菌属β—lase总检出率为80.4%,其中单独产青霉素酶、广谱酶、ESBLs菌株分别为7、4、21株,产ESBLs菌株总数32株(62.8%),未检出头孢菌素酶和碳青霉烯酶:产复合酶11株(21.6%),均产AmpC酶+ESBLs。(2)在4种产酶形式中,K.te检出率分别10.0%、10.0%、20.0%、20.0%;K.oz检出率分别5.6%、5.6%、33.33%、27.8%:K.ox检出率分别13.0%、8.7%、56.5%、17.4%。结论(1)本组51株克雷伯菌属产β-lase总检出率很高,其中K.ox最高;(2)4种产酶形式以AmpC酶、ESBI。S为主,未检出头孢菌素酶和碳青霉烯酶,各种克雷伯菌属菌所产各种酶的检出率不同。 Objective To investigate the distribution of β-1actamases produced by three species of Klebsiella spp. Methods β-lactamases produced by 51 strains of Klebsiella spp (K. te 10 strains,K. oz 18 strains,K, ox 23 strains) were detected with a multi disk test [synergy test,antagonize test for the inducible AmpC 13-1actamases (IABLs)]AmpC β-1actamases (ABLs) phenotype test and extented- spectrum β-lactamases (ESBLs) proving test. Results (1) The total positive rate of β-lactamases was 80.4%. In 50 strains, 7 strains produced penicillinase, 4 strains produced broad-spectrum β-lactamases, 21 strains produced ESBLs separately, totally 32 strains produced ESBLs (62. 8%), and all strains did not produce cephalosporinases and carbapenem-hydrolyzing β-1actamases (CHBLs); 11 strains produced both of AmpC and ESBLs; (2) In 4 types of producing β-lase,the detection rates of K. te were 10.0%, 10.0%, 20.0% and 20.0% respectively; the detection ratesofK, oz were 5.6%, 5.6%, 33.33% and 27.8% respectively; and the detection rates of K ox were 13.0%, 8.7%, 56.5% and 17.4% respectively. Conclusion (1) The total detection rate of β-lactamases produced by the 51 strains of Klebsiella spp is very high, and K. ox is the highest; (2) In 4 types of producing β-lase, AmpC β-1actamases and ESBI.s are the main ones, all don't produce cephalosporinases and CHBLs. The detection rates of all kinds of β lactamases produced by Klebsiella spp are different.
