慢性乙型肝炎患者血清可溶性E选择素和外周血T淋巴细胞亚群的变化及临床意义

Clinical Significance of Serum Soluble E-Selectin and T-Cell Subpopulations in Patients with Chronic Hepatitis B

目的探讨慢性乙型肝炎患者血清可溶性E选择素和外周血T淋巴细胞亚群变化及其临床意义。方法对50例慢性乙型肝炎患者和20例正常对照者,采用ELISA法检测血清可溶性E选择素(sE-selectin)水平,流式细胞术检测外周血T淋巴细胞亚群。结果与正常对照组相比,慢性乙型肝炎组sE-selection明显增高,差异具有统计学意义(P<0.01),慢性乙型肝炎中、重度患者外周血CD3、CD4、CD4/CD8比值明显降低,而CD8则明显升高,与正常人比较差异具有统计学意义(P<0.01);慢性乙型肝炎患者sE-selection与CD4/CD8比值之间呈负相关关系。结论检测血清E-选择素水平和外周血T淋巴细胞亚群对评估慢性乙型肝炎患者肝组织炎症和肝细胞功能损害程度、判断预后都有一定的价值。

Objective To evaluate the clinical significance of serum soluble E-selectin and T-cell subpopulations in patients with chronic hepatitis B. Method 50 patients with chronic hepatitis B and 20 normal control subjects were included in this study, E-selectin was measured by enzyme linked immunoserbent assay （ELISA）. The change of T-cell subsets was analyzed by flow cytometry. Result Compared with the normal control subjects, patients with chronic hepatitis B had a significantly higher serum E-selectin levels（P〈 0.01）. The levels of CD3, CD4, and the ratio of CD4 to CD8 were reduced in the sever cases of chronic HBV infection. The level of CD8 was also elevated （P〈0.01）. A negative correlation between the level of serum E-selectin and the ratio of CD4 to CD8 was observed. Conclusion Serum E-selectin and T-cell subpopulations may be used as indicators to indicate the liver damage in patients with chronic hepatitis B.
