Final Results of Delayed Pregnancy

目的分析延期妊娠的妊娠结局。方法选择2004年7月～12月入院的产妇，孕40^＋1．41^＋6周235例为观察组，孕37．40周252例为对照组，对其临床资料进行回顾性分析。结果观察组中胎盘Ⅲ^＋级50例，羊水过少33例，羊水Ⅱ°～Ⅲ°污染48例，胎儿窘迫45例，新生儿窒息32例，阴道助产86例，剖宫产90例，产后出血14例，其发生率明显高于对照组，差异有显著性意义（P〈0．05）。两组间死胎、死产、新生儿死亡、巨大儿、低体重儿及产褥感染发生率差异无显著性意义（P〉0．05）。结论延期妊娠时胎盘功能减退，羊水量减少，胎儿窘迫、新生儿窒息发生率增加，阴道助产、剖官产及产后出血机会亦增多。因此，必须加强孕期管理，核实孕周，积极采取措施及时终止妊娠，防止妊娠延期，以减少对母儿的危害。

Objective To analyze the final results of delayed pregnancy. Methods The puerperas were selected from July to December 2004, 235 cases of pregnancy for 40 ^＋1 ～ 41 ^＋6 weeks were taken as a group under observation, and 252 cases of pregnancy for 37 - 40 weeks as a group of contrast. A retrospective analysis about the clinical materials was made. Results For the former group, 50 cases were at placental maturity Ⅲ^＋ degree, 33 cases of oligohydramni os, 48 cases at amniotic fluid turbidity Ⅱ～Ⅲ degree, 45 cases of fetal distre s s, 32 cases of neonatal asphyxia, 86 cases assisted vaginal delivery , 90 cases of cesarean section and 14 cases of postpartum hemorrhage. The incidence was obvi ously higher than that of the contrast group. The difference between the two groups is obvious （P 〈 0.05） . As to fetal death, stillbirth, neonatal death, fetal ma crosomia, low birth weight infants and puerperal infections, there is no obvious difference （P 〉 0.05） . Conclusion When the pregnancy is prolonged, the functions of placenta as well as the amount of amniotic fluid will be reduced. The incidence of fetal distress and neonatal asphyxia will increase. And so will assisted vaginal deliveries, cesarean section and postpartum hemorrhage. As a result, in order to avoid pregnancy being delayed and its injury to the gravida and the fetus or the infant, we must reinforce the administration of the duration of pregnancy, make sure of the pregnancy weeks and take active measures to stop pregnancy.
