经阴道超声在恶性滋养细胞肿瘤的诊断及治疗中的应用

Applied Value of Transvaginai Ultrasound in Diagnosis and Treatment of Malignant Trophoblastic Tumors

目的:探讨经阴道彩色多普勒超声在恶性滋养细胞肿瘤诊断和治疗中的应用价值。方法:对21例恶性滋养细胞肿瘤的子宫动脉及肿瘤内的血流进行了检测,30例正常早孕组作为对照组。结果:恶性滋养细胞肿瘤的子宫动脉及肿瘤内血管的阻力指数明显低于正常早孕组,化疗前病变区血流异常丰富,呈现低阻性血流,化疗后病变区血流信号明显减少,化疗前后砌值明显有差异(P<0.05);监测化疗效果与血β-HCG相关性好。结论:经阴道彩色多普勒超声在滋养细胞肿瘤良、恶性的判断及化疗效果的监测上是一个很有价值的方法。

Objective To assess the value of Transvaginal Ultrasound in diagnosis and treatment of malignant trophoblastic tumors. Methods The uterine artery and intratumor blood flow were measured by color Doppler flow imaging（CDFI） in 21 patients with malignant trophoblastic tumor, with 30 cases of normal early pregnancies as control. Results The resistance indices （RI） of uterine artery and intratumor vessels in malignant trophoblastic tumor were significantly lower than those in controls, before chemotherapy, a colorful and rich low resistance flow spectrum was observed in the tumor area. After chemotherapy, the blood flow message decreased significantly （ P 〈 0.05 ）. Conclusion Transvaginal CDFI is useful for observing chemotherapeutic results of malignant trophobiastic tumors.
