ACEI在急性ST段抬高心肌梗死中的应用 被引量：2

摘要 血管紧张素转换酶抑制剂（ACEI）通过抑制肾素-血管紧张素系统（RAS），并作用于激肽酶Ⅱ，抑制缓激肽的降解．提高缓激肽水平．起到改善内皮功能．抑制炎症反应和血小板聚集的作用．并能稳定动脉粥样硬化斑块．改善心室重构。从而降低心脏事件和死亡率。已有大量临床试验的循证医学证据证明急性心肌梗死（AMI）发病36h之内口服ACEI使患者大大获益（Ⅱa，A），特别在前壁心肌梗死或射血分数减低或轻中度心力衰竭时（Ⅰ，A）。AMI后。有临床心力衰竭表现或无症状性左室功能不全患者应长期使用ACEI治疗（Ⅰ，A），高危或糖尿病患者也应长期接受ACEI治疗（Ⅰ，A）（见表1）。AMI后ACEI治疗的益处在糖尿病患者更为显著。
