可溶性转铁蛋白受体对类风湿性关节炎伴贫血疾病的诊断价值

The value of serum soluble transferrin receptor in the diagnosis of anemia of rheumatoid arthritis

目的探讨外周血可溶性转铁蛋白受体(sTfR)水平对类风湿性关节炎(RA)伴贫血疾病的诊断价值。方法采用酶联免疫吸附实验(ELISA)方法检测105例RA伴贫血患者(RA伴IDA患者73例,RA伴ACD患者32例)和健康体检者(健康对照组)168例的sTfR的血清水平,同时测定血红蛋白(Hb)、血清铁蛋白(SF)和转铁蛋白(Tf)水平。结果sTfR,Hb,SF和Tf在RA伴贫血患者组、RA伴IDA患者组、RA伴ACD患者组和健康对照组间差异有显著性(P<0.01)。对于sTfR而言,健康对照组与RA伴ACD组间差异无显著性(P>0.05)外,其他组间差异均有显著性(P<0.01),且RA伴IDA组血清sTfR水平明显高于RA伴ACD组(P<0.01)。结论sTfR血清水平的升高有助于RA伴贫血的诊断,并且在鉴别RA伴贫血类型的诊断中有一定的参考价值。

Objective To study the diagnostic value of serum soluble transferrin receptor （sTIR） for anemia of rheumatoid arthritis （RA）. Methods Serum level of sTIR was measured by ELISA in 105 RA patients. Results There was significant difference in serum level of sTfR between anemia of RA group and non - anemia of RA group （ P 〈 0.05 ）. And the serum level of sTIR with iron deficiency anemia （ICD） of RA patients group was significantly higher than that of the anemia of chronic disease （ACD） of RA patients group （P 〈0.05）. Conclusion High level of sTfR is helpful in the diagnosis of anemia of RA. Moreover, sTfR is also useful for the differential diagnosis of ICD and ACD of RA.
