异丙酚复合咪唑安定对正中神经中短潜伏期体感诱发电位的影响

摘要 目的在BIS监测下,研究咪唑安定异丙酚麻醉对正中神经中短潜伏期体感诱发电位(SSEP)的影响。方法择期手术患者30例,随机分为两组(n=15):咪唑安定复合异丙酚组(M组)、异丙酚组(P组)。M组在靶控异丙酚前静脉注射咪唑安定0.04mg/kg,P组单纯靶控异丙酚。靶浓度从1μg/kg渐升至3μg/kg。在清醒、BIS值为70,50及小于40时记录正中神经中短潜伏期诱发电位。结果与清醒比较,BIS值为50及小于40时,P组N20、N35波潜伏期明显延长(P<0.05),M组无明显变化;两组N55波潜伏期均明显延长(P<0.05)。BIS值为70时,两组N35-P45波幅均明显降低(P<0.05,P<0.01);P组P45-N55波幅明显降低(P<0.01),M组无明显变化。BIS值50及小于40时,两组N20-P25及P25-N35波幅无明显变化;N35-P45,P45-N55波幅均明显降低(P<0.05,P<0.01)。结论单纯应用异丙酚麻醉明显抑制中短潜伏期SSEP,异丙酚复合咪唑安定麻醉只抑制中潜伏期SSEP,对短潜伏期SSEP影响小。
