口腔科消毒效果影响因素的Logistic分析

AN ANALYZING LOGISTIC STUDY ON POTENTIAL FACTORS INFLUENCING THE EFFECT OF DISINFECTION IN ORAL CLINICS

目的分析医院口腔科和口腔诊所消毒监测结果,探讨影响消毒效果的因素,找出主要的危险因素,切断可能引起口腔医院感染的传播途径,提出控制口腔医院感染的有效措施。方法选取82家具有卫生主管部门批准的口腔医疗许可的医院口腔科和口腔诊所。通过调查表收集2004年1月～2006年3月期间被监测单位影响口腔科消毒效果的因素(自变量),收集被监测单位消毒效果监测结果,主要包括空气、物体表面、医护人员手及无菌消毒液污染监测等四项消毒效果的监测结果(因变量),并进行分析。结果诊疗用房面积大小、诊疗设备多寡、消毒方式、医院是否有严格的消毒管理制度以及医务人员的教育素质都直接影响口腔科消毒效果。口腔诊所消毒效果、人力配备和感染控制制度都低于医院口腔科。结论适当扩大医疗用房面积,保证污染区、清洁区、休息区的划分,严密消毒制度,可防止医院内感染,提高医疗效果。医护人员重视自我防护,认真执行消毒工作,可降低物体表面的细菌污染,从而提高消毒效果。医护人员必须认真、规范洗手。保证使用中消毒液的浓度及正确使用是防止医院内感染的重要措施之一。

Objective To explore the influencing factors of disinfections as well as to determine the dangerous sub- factors applying binary logistic regression analysis method, and provides some effective measures for the control of the cross - infection in oral clinics. Methods Selects 82 oral clinics at the general hospitals and dental clinics that have the health supervisor section approval admit. Collects the questionnaire during Jan. 2004 to Mar. 2006 and the outcome of inspecting air sample, the degree of bacilli contaminated on the objects, asepsis instruments sample and the hands of doctors and nurses, then carry on the analysis. Results The disinfections effect in oral clinics will be directly influenced by the size of clinic area, the amount of medical equipment, the method of disinfections, the system of controlling cross - infection in hospitals as well as the personnel＇s educational degree on dentistry. The disinfections effect human resource and cross - infection control system in dentist clinics are all worse than in general hospital clinics. Conclusion The appropriate extension of the building area for medical treatment use, assurance pollution area, sweep the area and take a rest the demarcation of the area, tight disinfections system, can prevent from the infection inside the hospital, raise the medical treatment result. Medical personnel defend the protection by one＇s own, the earnest performance disinfects work, can lower the germs pollution of the object surface, and thus raise the disinfections result. Medical personnel must wash hands according to standard. Guarantee of the density and correct use of disinfections liquid is one of the important measures to prevent from nosocomial infection.
