改良乙状窦前入路切除岩斜区肿瘤

A modified transpetrosal-presigmoid approach to petroclivus lesions

目的介绍一种新的、改良的乙状窦前入路的开颅方法。方法对124例岩斜区肿瘤,通过颞枕小脑幕上下双骨瓣开颅、改良乙状窦前入路手术方式,探讨传统乙状窦前入路和改良乙状窦前入路的优缺点。结果124例改良乙状窦前入路,手术中无静脉窦破裂出血的发生,术后无颅骨缺损、局部积液的发生。手术操作安全、简便、易于掌握。结论改良乙状窦前入路在传统入路基础上,通过对开颅骨瓣的改良,避免了手术中静脉窦损伤、手术后颅骨达到完全解剖复位,不仅使局部美观,还有效地降低了术后并发症的发生。

Objective To introduce a modified manipulation of presigmoid sinus approach. Methods A series of 124 cases of petroclival tumors were treated with modified presigmoid approach of double bone flaps. It＇s advantage and disadvantage were evaluated with the traditional methods. Results In the modified approach, there was no rupture or bleeding of the sinus, no bone defect, and no subcutaneous collection. Conclusion The modified transpetrosal-presigmoid approach has several predominant advantages such as avoiding sinus injury, preventing bone defect, and decreasing postoperative complications significantly.
