慢性肾功能衰竭血浆P物质、心钠素与肾功能的关系

The relationship between plasma P substance、 atrial natriuretic peptide and renal function in chronic renal failure

目的了解慢性肾功能衰竭（ＣＲＦ）患者血浆Ｐ物质（ＳＰ）、心钠素（ＡＮＰ）含量变化与肾功能的关系。方法采用放射免疫分析法检测正常对照组及ＣＲＦ组血液透析前后ＳＰ、ＡＮＰ，同时测定血清肌酐（Ｓｃｒ）。结果（１）透析前ＳＰ显著高于正常对照组（Ｐ＜００１），透析后ＳＰ高于透析前（Ｐ＜００５）；ＳＰ透析前与Ｓｃｒ呈正相关（ｒ＝０６６，Ｐ＜００５），ＳＰ透析后与Ｓｃｒ呈负相关（ｒ＝－０４６，Ｐ＜００５）；（２）透析前ＡＮＰ显著高于对照组（Ｐ＜００１），透析后ＡＮＰ显著低于透析前（Ｐ＜００５），ＡＮＰ透析前后与Ｓｃｒ呈正相关（ｒ＝０８２，Ｐ＜００５）；（３）透析前后ＳＰ变化值与ＡＮＰ变化值呈负相关（ｒ＝－０４８，Ｐ＜００５）。结论ＣＲＦ患者血浆ＳＰ、ＡＮＰ含量的变化与肾功能的改变有较明显的相关性。

Objective To investigate the relationship between plasma P substance (SP)、 atrial natriuretic peptide (ANP) and renal function in chronic renal failure (CRF). Methods SP and ANP were measured by radioimmunoassay in normal control and CRF before and after hemodialysis (HD), serum creatinine (Scr) was determined simultaneously. Results (1)SP before HD was significantly higher than that of control (P<0 01) and significantly lower than that after HD (P<0 05). There was a positive correlation between SP before HD and Scr(r=0 66, P<0 05), a negative correlation between SP after HD and Scr (r=-0 46,P<0 05). (2)ANP before HD was significantly higher than that of control (P<0 05). There were positive correlations between Scr and ANP before and after HD (r=0 82,P<0 05). (3)There was a negative correlation between the quantitative difference of SP and that of ANP before and after HD (r=-0 48,P<0 05). Conclusion There is a significant correlation between the change of plasma SP、ANP concentrations and Scr level in CRF patients.
