Ⅰ期非小细胞肺癌中肝癌衍生生长因子的表达及其临床意义

Expression of Hepatoma-Derived Growth Factor and Its Clinical Implication in Stage Ⅰ Non-Small Cell Lung Cancer

摘要 背景与目的肝癌衍生生长因子(hepatoma-derivedgrowthfactot,HDGF)是近年来逐渐引起重视的一种细胞生长因子。研究表明HDGF在多种正常细胞和肿瘤存在高表达,能够促进细胞增殖、组织分化以及血管生成.与多种恶性肿瘤的侵袭、转移及其预后相关。本研究旨在分析HDGF在Ⅰ期非小细胞肺癌中的表达及其临床意义。方法应用免疫组化SP法检测118例Ⅰ期非小细胞肺癌以及30例正常肺组织中HDGF的表达。结果HDGF在胞浆和胞核均有表达.在肺癌组织中表达普遍上调,正常肺组织HDGF评分52.23±10.35,肺癌组织评分为156.73±70.95,差异有显著性(P〈0.01);HDGF表达与病理类型显著相关,而与其他临床病理因素均无明显相关性(P〉0.05),腺癌HDGF评分明显高于鳞癌,差异有显著性(P=0.001);HDGF高表达组5年生存率较低表达组明显降低,分别为40.0%和77.5%,差异有显著性(P=0.008)。多因素生存分析结果表明HDGF为Ⅰ期非小细胞肺癌的独立预后指标(RR=1.011,P=0.002)。结论HDGF可以作为1期非小细胞肺癌患者的独立预后指标,对HDGF的深入研究可能为I期非小细胞肺癌的发生、发展提供新的视角。 Background and objective Hepatoma-derived growth factor (HDGF),a novel growth factor, has a widely expression in many normal cells and tumor cells. It plays an important role in cell proliferation,differentiation and angiogenesis.lt is eonsiered as a promising marker for predicting the invasion,matastasis and prognosis of carcinomas in clinical researches.The aim of this study is to evaluate the expression of HDGF and its clinical implication in patients who undergone complete resection for stage I non-small cell lung cancer (NSCLC).Methods Immunohistochemieal technology was applied to detect the expression of HDGF in 118 lung cancer tissues and 30 normal lung tissues as control.Results HDGF staining was observed in nuclear as well as in eytoplasm.HDGF positively staining was seen in all patients, and remarkably higher than that in normal lung tissues (52.23±10.35 vs 156.73±70.95, P〈0.01).Expression of HDGF was closely related to histological classification,hut not to other elinieopathologieal factors,and the expression of HDGF in adenocarcinoma was much stronger than that in squamous eaneers(P=-0.001).Univariate anal- ysis and multivariate Cox regression analysis showed that the patients with high HDGF expression had a shorter overall survival and HDGF was a significantly independent predictive factor for patients with stage ⅠSCLC (RR=1.011,P=0.002).Concluslon HDGF may be a promising predictive factor for stage Ⅰ NSCLC,and the assessment of HDGF may provide new insight on carcinogenesis and development of stage ⅠNSCLC.
