Preventing effect of the compound of transforming growth factor-β_1 antibody with fibrin glue (TGFβ_1 Ab + FG) on postoperative adhesions in the chicken's flexor tendon in zone Ⅱ(no man's land)

摘要 目的:研究TGFβ1抗体复合生物蛋白胶(FG)预防鞘管区屈肌腱粘连的作用和对肌腱愈合的影响.方法:将72只来亨鸡(leghorn)按随机化原则分为单纯TGFβ1抗体(TGFβ1Ab)组、单纯生物蛋白胶(FG)组、TGFβ1抗体复合FG(TGFβ1Ab+FG)组及生理盐水(NS)组,每组各18只.分别将鸡左足第3,4趾趾深屈肌腱横断,作The4-strandCruciaterepair缝合,在损伤周围注入药物.术后5d行早期功能锻炼,在第1,3,8wk处死取材,分别进行大体观察、组织学检查、生物力学测试.结果:TGFβ1Ab复合FG组术后3,8wk,缝合处粘连程度评分、肌腱滑动距离及模拟主动屈曲度均优于其余三组,且差异有统计学意义(P<0.05).四个处理组的最大抗断裂载荷差异无统计学意义(P>0.05).结论:TGFβ1抗体复合生物蛋白胶可以有效预防术后肌腱粘连,不影响肌腱的正常愈合进程. AIM: To study the preventing effects of the compound of transforming growth factor-β1 antibody with fibrin glue (TGFβ1 Ab + FG) against of peritendinous adhesive formation, METHODS: The long flexor tendons of the third toes in zone Ⅱ of 72 chickens (leghorn) were transversed and sutured with the 4-strand Cruciate repair technique. After operation, the chickens were divided into 4 groups. The compound mentioned above was injected around the repaired tendon in one group. TGFβ1 antibody, FG and normal saline (NS) were used in the other 3 groups respectively. Chickens from each group were sacrificed at 1, 3 and 8 weeks after operation. The results were evaluated grossly, histologically and biomechanically. RESULTS: The gliding function of chickens, toe treated with the compound of TGFβ1 with FG was superior (P 〈 0.05 ) to that treated with NS at 3, 8 weeks after operation. The gross appearance of injury site differed clearly ( P 〈 0.05 ) between the 2 groups. Chickens treated with TGFβ1 antibody and FG had the same effects as those treated with NS. No significant difference in tendon anastomosis breaking strength was observed among the 4 groups ( P 〉 0.05 ). CONCLUSION: The compound of TGFβ1 antibody with FG could significantly control the formation of fibrous adhesions without interfering the healing process.
