妊娠期糖尿病的临床治疗与妊娠结局的关系探讨

Discussion on Relationship Between Clinical Treatment of Gestational Diabetes Mellitus and Pregnancy Outcome

目的探讨妊娠期糖尿病(GDM)的临床治疗,并分析与妊娠结局的相关性。方法比较我院90例经收入院治疗的GDM孕妇和90例同期因各种原因未得以收入院干预治疗的GDM孕妇的临床资料,分析孕妇的分娩方式、产科并发症及预后等情况。结果治疗组新生儿窒息、低血糖、围产儿死亡、巨大儿、羊水过多、产后出血、妊高征、胎膜早破方面及分娩方式与对照组比较,差异有统计学意义(P<0.05)。结论GDM应早期诊断和治疗,控制血糖使血糖维持在正常水平是GDM治疗的关键,必要时加用胰岛素、适当放宽剖宫产指征,有利于获得良好妊娠结局。

Objective To study the correlation between clinical treatment of gestational diabetes mellitus （GDM） and the related pregnancy outcomes. Methods The clinical data of 90 GDM pregnant women treated in our hospital and 90 GDM pregnant women without hospitalized interference treatment were compared; and the way of parturition, obstetric complications and prognosis of the two groups were analyzed. Results Infant asphyxia, low blood sugar, perinatal deaths, fetal macrosomia, excessive amniotic fluid, postpartum hemorrhage, pregnancy- induced hypertension syndrome, premature delivery, preeclampsia, premature rupture of membranes, postpartum bleeding and mode of delivery of the treatment group were significantly better as compared with the control group （P〈 0.05）. Conclusions GDM should be early diagnosed and treated. Control of blood glucose level is the key in treatment of GDM, administration of insulin and appropriate relaxation of cesarean section indication can conduce to good pregnancy outcome.
