摘要
目的研究椎体前复合组织瓣移位缝合生物固定预防单节段颈椎椎体间植骨块滑脱的治疗效果。方法颈前常规入路,于椎前筋膜及前纵韧带设计Z字形组织瓣,同时在植骨块两侧取颈长肌的内侧部,在颈交感干的内侧缘纵行设计双蒂颈长肌肌瓣,于植骨块前方将复合组织瓣移位缝合同定以预防单节段颈椎椎体间植骨块的滑脱。结果共治疗218例,颈椎融合率达到98.5%。3例经CT扫描为可疑不全融合,但未出现症状,未出现植骨块突出、滑脱等移位现象。结论椎体前复合组织瓣移位缝合生物固定预防单节段颈椎椎体间植骨块滑脱,可获得牢固可靠的生物同定,有效避免植骨块的滑脱。
Objective To study the effect of displacing and suturing complex tissue flap in front of centrum of vertebra on preventing the slip of bone grafting the cervical anterior decompression. Methods Through cervical anterior approach, we designing "Z" tissue flap on the fascia prevertebralis and anterior longitudinal fascia , in the same time, designed double stalk longus collis muscl beside of grafting bone, and sutured complex tissue flap. Results There were no slip of grafting bone in all the 218 patients, the rate of bony union was 98.5%. Conclusions Displacing and suturing complex tissue flap in front of centrum of vertebra can prevent the slip of bone grafting the cervical anterior decompression, and can obtain dependable and firmly biological fixation.
