疾病不确定感对初诊鼻咽癌患者放疗反应、癌因性疲乏及生活质量的影响

Influence of Disease Uncertainty on Side Effects of Radiotherapy,Cancer-related Fatigue and Quality of Life Among the Newly Diagnosed Nasopharyngeal Cancer Patients

目的探讨疾病不确定感对初诊鼻咽癌患者放疗反应、癌因性疲乏及生活质量的影响。方法对135例初诊鼻咽癌接受常规放疗患者,采用疾病不确定感量表、RTOG/EORTC放射性损伤评估量表、癌因性疲乏评估量表和世界卫生组织生存质量简表进行问卷调查。以疾病不确定感平均得分分为高分组(73例)和低分组(62例),比较两组放疗反应、癌因性疲乏及生活质量。结果初诊鼻咽癌放疗患者疾病不确定感平均得分为119.57±31.80。低分组放疗反应(除黏膜反应、皮肤反应外)、癌因性疲乏、生活质量得分显著优于高分组(P<0.05,P<0.01)。结论初诊鼻咽癌患者疾病不确定感较严重,影响其放疗反应、癌因性疲乏及生活质量,需采取有效措施降低患者的疾病不确定感,提高生活质量。

Objective To investigate the influence of disease uncertainty on side effects of radiotherapy, cancer-related fatigue and quality of life in the patients with newly diagnosed nasopharyngeal cancer. Methods One hundred and thirtyfive newly diagnosed nasopharyngeal cancer patients were surveyed by Medical Uncertainty Inventorg Scale, RTOG/ EOTRC Radiation Damage Scale, （EORTC） QLQ-C30 questionnaire about cancer-related fatigue evaluation and WHO- QOL-BREF questionnaire, respectively. By the average score of disease uncertainty, the patients were divided into high score group （73 cases） and low score group （62 cases）, and the differences in side effects, cancer-related fatigue and quality of life were compared between the two groups. Results The mean value of disease uncertainty was 119.57± 31.80. The scores in the side effects, the cancer-related fatigue, and quality of life were significantly lower in the low score group than those in the higher score group （P〈 0.05, P〈 0.01 ）. Conclusion The disease uncertainty of newly diagnosed nasopharyngeal cancer patients were severe and can influence the side-effects of radiotherapy, cancer-related fatigue and quality of life. Nurses should take effective interventions to decrease patients＇ uncertainty to improve their quality of life.
