盐酸哌替啶联合米索前列醇用于终止早期妊娠人工流产的临床分析

Clinical Analysis of the Application of Pethidine plus Misoprostol in Induced Abortion of Early Pregnancy

目的观察盐酸哌替啶联合米索前列醇用于终止早期周妊娠人工流产术中的效果。方法对100例早孕妇女术前3～4小时置米索前列醇200mg于阴道穹窿处，3小时后在消毒的同时予以静脉注射盐酸哌替啶1.5mg／Kg。观察镇痛效果、官口松弛程度、术中出血量、人工流产综合反应发生率、宫颈损伤情况、手术时间、术后离院时间。结果盐酸哌替啶联合米索前列醇用于人工流产术中的镇痛效果明显，有效率达98％，与对照组比较，无显著差异（P〉0．05）。宫口松弛程度与对照比较组比较有显著性差异（P〈0．01）。术中出血量与对照组比较有显著性差异（P〈0．01）。人工流产综合反应发生率与对照组比较有差异（P〈0．05）。手术时间与对照组比较有显著性差异（P〈0.01）。宫颈损伤情况与对照组比较有显著性差异（P〈0.01）。术后离院时间与对照组比较无显著性差异（P〉0．05）。结论盐酸哌替啶联合米索前列醇用于人工流产术中能达到良好的镇痛作用，有效减少人流并发症。术前进行宫颈准备和术中应用适当的镇痛方法，有助于提高人工流产术的安全性，降低患者的经济负担。

Objective To observe the effect of the application of pethidine plus misoprestol in induced abortion of early pregnancy. Methods 100 early pregnant women received the compound application of pethidine and misoprestol.200mg of misoprostol was put in the fomix uteri 3 to 4 hours before the abortion and an intravenous injection of 1.5mg/Kg of pethidine was given 3 hours later. Observations were made to analgesic effect, the level of incompetence of internal orifice of uterus, the amount of intraoperative bleedirg, incidence of comprehensive reaction of induced abortion, the cervical injury, the operative time and postoperative discharge time. Results The compound application of pethidine and misoprestol in induced abortion is of obvious analgesic effect, the effective rate reached 98 %, the comparison with the control is of no significant difference （ P 〉 0.05 ）. There existed a significant difference when compared with the control in the level of incompetence of internal orifice of uterus （ P 〈 0.01 ） ,the amountof intraoperative bleeding （ P 〈 0.01 ）, the general reaction to induced abortion （ P 〈 0.05 ）, the operative time （ P 〈 0.01 ） and the cervical injury （ P 〈 0.01 ）, no significant difference was found in postoperative discharge time （ P 〈 0.05 ）. Conclusion The compound application of pethidine and misoprostol in induced abortion is of good analgesic effect and can reduce the incidence of complications effectively.
