坐骨神经结扎大鼠脑脊液补体异常活化与痛觉过敏的关系 被引量:8

Relationship between Abnormal spinal complement activity and hyperalgesia in rats following sciatic nerve ligation

摘要 目的观察坐骨神经慢性压迫性损伤(chronic constriction injury,CCI)后补体的变化特点,探讨脊髓补体异常活化与大鼠痛觉过敏的关系。方法健康雄性SD大鼠40只分为假手术组、CCI组、生理盐水组和CVF组,对CCI组、生理盐水组和CVF组大鼠左侧坐骨神经进行松结扎,而假手术组只暴露左侧坐骨神经但不予任何处理,CVF组大鼠在术后腹腔注射眼镜蛇毒因子(CVF)进行干预,生理盐水组大鼠在术后腹腔注射等量的生理盐水作为对照CCI组。于术前和术后1—7d每天测定大鼠的热痛阈值,采用免疫比浊法测定4组大鼠血清和脑脊液补体C3含量,并在第7天处死大鼠取L4-L6段脊髓,匀浆后测其补体C3含量。结果(1)CCI组大鼠在坐骨神经结扎后出现热痛觉过敏,而假手术组大鼠则无此变化,给予生理盐水并不能影响坐骨神经结扎后出现的热痛敏感现象,而给予CVF却能抑制CCI大鼠热痛敏感的发生;②假手术组、CCI组及生理盐水组大鼠血清补体C3含量手术前后无统计学差异;与术前相比,CCI组和生理盐水组术后大鼠脑脊液补体C3浓度升高显著(P〈0.01),CVF组大鼠血清和脑脊液补体C3含量明显减低;③术后第7天,CCI组大鼠脊髓中补体C3含量显著高于假手术组,生理盐水组大鼠脊髓中补体C3含量与CCI组相当,CVF组明显低于CCI组;④大鼠热痛阈值与血清补体含量无相关性,与脑脊液补体含量呈负相关。结论外周神经损伤后,中枢神经系统存在补体异常活化现象,补体的这种变化特点与痛觉过敏的形成密切相关。 Objective To observe the dynamic changes of complement protein 3 in rats with the chronic constriction injury (CCI) of the sciatic nerve and investigate the relationship between spinal complement activity and hyperalgesia. Methods Forty healthy male Sprague-Dawley rats were randomly divided into sham-operation group, CCI group, saline group and CVF group. The left sciatic nerve was ligated loosely according to Wang Jin-bao' s method in all rats except those underwent sham operation. The rats of CVF group were intraperitoneally injected with cobra venom factor after CCI, while the rats of saline group with saline as control. Pain threshold by thermal stimulation was recorded and the level of complement protein 3 in serum and cerebrospinal fluid (CSF) were determined by the immunoturbidimetry preoperatively and everyday in postoperative seven days, the levels of complement 3 in spinal cord were determined on postoperative day 7. Results Thermal hyperalgesia developed after sciatic nerve ligation in CCI group, but not in sham-operation group. Cobra venom factor but not saline relieved the hyperalgesia. The level of complement 3 in serum in sham group, CCI group and saline group showed no changes. The postoperative levels of complement 3 in CSF of CCI group and saline group was significantly higher than that before operation ( P 〈 0. 01 ). The postoperative level of complement protein 3 in serum and CSF in CVF group exhibited lower than that before operation. The level of complement protein 3 in spinal cord was higher obviously in CCI group and saline group than sham group on postoperative day 7 ( P 〈 0. 05 ), lower in CVF group than CCI group. The thermal hyperalgesia was correlated with the level of complement in spinal cord but not with that in serum. Conclusion The complement compo nerve system are activated abnormally after peripheral nerve injury, and the dynamic changes may have an intimate relation with the formation of hyperalgesia.
