摘要 目的:探讨白内障合并超高度近视眼行超声乳化吸除联合低度数人工晶状体植入术的临床疗效。方法:表麻下对30例37眼超高度近视合并白内障患者行超声乳化白内障吸除联合低度数人工晶状体植入术。术后观察患者最佳矫正视力、屈光度数和手术并发症。随访6~12mo。结果:术后最佳矫正视力≥0.3者共22眼(59%),≥0.5者共12眼(32%)。术后屈光度数偏差值<±0.50D共23眼(62%);±0.50D<术后屈光度数偏差值<±1.00D共12眼(32%),±1.00D<术后屈光度数偏差值<±2.00D共2眼(6%)。术中仅2眼晶状体后囊膜破裂,术后1眼出现双眼干扰症状,后囊混浊2眼。8例患者术后第1d角膜轻度水肿,在3~5d内自行消退。无视网膜和脉络膜脱离者,无眼压升高者。结论:超声乳化联合低度数人工晶状体植入术治疗超高度近视白内障患者,能有效改善患者的视力。 AIM: To evaluate the clinical effect of phacoemulsification and low power intraocular lens implantation for super high myopia with cataract. METHODS: Phacoemulsifiaction and low power intrao-cular lens implantation were performed on 37 eyes of 30 patients with super high myopia. The best corrected visual acuity, diopter and complications after the operation were observed. Follow-up period was 6-12 months. RESULTS: Postoperative best corrected visual acuity was ≥0.3 in 22 eyes (59%), ≥0.5 in 12 eyes (32%). The deviation value of diopter after the surgery was 〈 ± 0.50D in 23 eyes(62%), more than ±0.50D but less than ±1.00D in 12 eyes (32%), and more than ±1.00D but less than ± 2.00D in 2 eye (6%) . Posterior capsule rupture occurred in 2 eyes; one patient had the symptom of binocular interference after the operation; posterior capsular opacification occurred in 2 eyes. Corneal edema was found in 8 eyes on the first day after the surgery, and it disappeared within 3 to 5 days. None of the eyes were found with retinal detachment, choroidal detachment or ocular hypertension. CONCLUSION: Phacoemulsifiaction and low power intra-ocular lens implantation can significantly improve visual acuity in patients with super high myopia and cataract.
