摘要
目的探讨肥大细胞(MC)在乳腺导管内上皮瘤变(DIN)与浸润性导管癌中的表达及其对该2类疾病诊断及远期预后判断的意义,并探讨该2两类患者发现年龄方面的变化状态,以揭示其流行病学特点。方法MC检测用MC和胶原纤维组合染色法,组织学分级用Patley和Scarff方法。结果2类75例患者(DIN20例、乳腺浸润性导管癌55例)MC数量差异有统计学意义(H=12.08,P<0.01);55例乳腺浸润性导管癌中的Ⅰ、Ⅱ、Ⅲ级患者的MC数量差异有统计学意义(H=19.12,P<0.01)。2类75例患者的病变发现年龄差异有统计学意义(H=16.68,P<0.01)。结论MC数量与组织学分级结果,存在明显的对应关系,即两类患者的MC数量是DIN>乳腺浸润性导管癌;乳腺浸润性导管癌中Ⅰ、Ⅱ、Ⅲ级患者的MC数量是Ⅰ>Ⅱ>Ⅲ,差异有统计学意义(H=9.12,P<0.01)。说明MC数量可作为该两类疾病诊断及远期预后判断的参考指标。
[ Objective] To observe the expression of mass cells ( MC ) in the mammary ductal intraepithelial neoplasia (DIN) and invasive ductal carcinoma, the significance of diagnosis and prognosis of these two diseases, and the changes in age in order to discover the epidemiological features of them. [ Methods] Mass cells and collagen fibers compound staining method was used to detect the mass cells; Patley and Scarff were applied for the histological grading. [ Results] The quantity of the mass cells in the ductal intraepithelial neoplasia was significantly higher than that of the invasive ducatl carcinoma ( P 〈0.01 ) ; significant difference was also found in the quality of the mass cells between the three types of invasive ductal carcinoma ( P 〈0.01 ) as well as the age between the two diseases; [ Conclusion] Corresponding relations exist between the quantities of the mass cells and the histological grading, which could be used as the valuable indexes for the diagnosis and long-term prognosis of the two diseases.
